What Forms are for

Forms are used to collect requests from people (website visitors, Instagram leads, referrals) and turn them into structured data inside your CRM.

They are perfect when you want:

Fewer back-and-forth messages
Consistent info from every inquiry
Automatic creation of clients and projects/leads

New form

Creates a new form. You set:

A name (what you call it in your dashboard)
An optional description (shown on the public page)

Publish / Unpublish

Publish makes the form available publicly (so people can submit it).
Unpublish hides it from the public.

Important rule (practical meaning):
To publish, you must have a required email field in your form. That’s because Mosaiico needs a reliable way to identify the person submitting.

Open public form

Only available when the form is published. It opens what the visitor will see.

Open builder

Takes you to the editor to manage fields and design.

Submissions

Takes you to the submissions inbox (everything people sent).

Form Builder — everything you can configure

A) Form “content” (title & description)

This is what visitors see at the top of the public form page.
Use it to set expectations (example: “Tell me about your event and I’ll reply within 24h”).

B) Fields (questions people fill in)

You can create different question types (email, phone, text, select, checkbox, etc.). For each field you can set:

Label: what the user reads
Required: whether they must answer to submit
Order: drag-and-drop priority

The Email rule (what it really means)

Your form must have one email field that’s required.
This email is used to:

Find or create the client automatically
Connect the submission to a real person
Let you follow up

C) Styling (how the form looks)

You can change colors, typography, spacing, etc. to match your brand.
This is especially useful when you embed the form on your website.

D) Preview

Shows you what the form will look like when published.

Submissions — how to use it like an inbox

Submissions is your “inbox” for forms.

Filters

You can filter by:

Form
Date range

Submission list (what you see)

Each row shows:

When it was submitted
Which form it came from
The email (so you can recognize the person quickly)

Open a submission (why it’s useful)

When you open a submission you see:

All answers in a readable layout
In the same order as your form fields

What happens automatically when someone submits a form

When a visitor submits:

The submission is saved.
If an email is present:
- Mosaiico finds/creates a client.
A new lead/project can be created from the submission.
You get a notification (and may receive an email alert).

This is how Forms become a “lead capture machine”.

Common mistakes to avoid

Making too many fields required (people abandon)
Not asking anything that helps you price (you still need a call)
Publishing without testing the form yourself once
